Insert template into document

I have an MS Word templated contract that is populated via macros.  The signature page has a number of fields that needs to be filled in by various signers.  This signature page is consistent across the specific contracts.  I have created a NitroSign template of just the signature page.  I'd like to insert the NitroSign templated signature page into the MS Word document that I have uploaded and send the entire document for signing.

When I try to use the 'combine' feature of NitroSign, I can see the Word doc, but I can't figure out how to get the signature page into the document.  The signature page doesn't show up as a file available to combine.

Is it possible to insert the NitroSign templated signature page into the MS Word document that I have uploaded?
