Device selection box does not present when I go the page where deactivation occurs.

I am trying to deactivate a specific serial number but, when I enter the number and click the DEACTIVATE button, a box appears asking me to select a device.  But. there is NO box on the page to do that.   I have only one device, my laptop, but there is nothing on the page to deselect a device once I have searched for a specific serial number.  How can this be fixed?
