Icon with Question Mark

I have users certifying PDF files and alongside the signature they are now getting a little icon of a man with a question mark. I am not aware of any changes having been made.

What does this mean & how can we get rid of it?

This icon shows up with signed documents in a particular situation:

I am not sure what is meant by "and the current view of the document is not the same as when it was signed". Please clarify.

Hi Code,

 

"The current view of the document is not the same as when it was signed" means that there was some changes/additions/subtractions/edits made to the document, after the Digital Signature was placed there. 

This particular icon is showing that the Digital Signature was not validated initially and has had changes made to it after being added to the PDF file.
