2D Barcode Disapears

I am creating a basic simple form containing a list box, text field checkbox along with a barcode field.  When I open the form in Nitro 5 reader and start filling out the form, the barcode disapears.  Not sure what I am missing here. 

I have tried value as TabDeliminated, and XML and the same behavior occurs.

Anyone have any ideas?
