Upgrade & Consolidate Licesnes

We have  a small number of licenses for versions 9 and 10 spread over 3 different serial numbers. Is it possible to consolidate everything under a single serial number, upgrade to the latest version and purchase software assurance?

We tried using the online upgrade tool and one of our licenses produced the error "There's been an error with your submission. Please head to our Community Forum and update us with your issue."
