Cannot edit Text Field Names

Hello, I have Nitro Pro 12.12.1.522 and when I try to edit a fillable text field name in an existing document, it changes the field type to check box.  This also happens when I try to copy the field (it changes to check box rather than text field).

Thanks Leslie.  How do I upload the file?  When I click on insert other media, existing attachment it tells me that I have no attachments and I can't determine where to upload one.  Thanks.
