digital signature

I have added the digital signature box to my pdf but it is not working.  I do not know what I am doing wrong and have read the tutorials over and over.  Does anyone have any suggestions?

Hi I have since learned this will not work for what we are trying to accomplish which is to put the form on our website so it can be digitally signed.  This is my 3rd response to you and I have heard nothing back so I will assume you're not able to help?????????????????????
