Unable to optimize, compress large heavy PDF file

I was unable to reduce a heavy file size pdf, compress like i was able to compress a pdf file with Adobe Pro as i have tried. the nitro pdf pro 13 does not really compress a heavy pdf file for example a pdf file size: 3096KB was compressed by adobe pro and file reduced to size: 644KB. which is what i really need in order to attach the file online in the medical system. As attachments in various systems and uploading to a limit of 1mb.

i hope there is a solution or there can be
