Redaction causes large black rectangles on the document.

Good morning, I am having issues when redaction parts of documents where I highlight the lines to redact, and then click on the Apply all Redactions button to black out the selected areas.  After that areas around the selection also become redacted as well.  It is not on every document but many of the ones we use it does happen to.  This is happening on both Nitro 12 and 13 machines but not ones with Adobe Acrobat DC.  Any help in resolving the issues would be greatly appreciated.
