Error/Request not sent

We are experiencing error in sending request for signature. The second time we try to send the document the notification will be "Request not sent, This document may already be out for signing" but in reality it is not send and still on draft status. In addition the email address of recipients became "?" and when attempting to send again the notification will say "no recipients". Please help.
