Create form fields for document

With Nitro PDF Pro for Mac this feature does not work.  You only get the option to "Create form fields for Page..." no matter what I do for example:

1. highlight the document (command+A) and tried edit and all you see is the option to "Create form fields for Page..." 
2. Selected both pages on the sidecar and tried edit and all you see is the option to "Create form fields for Page..." 

The only way to get the next page is to go to it and do, the option to "Create form fields for Page..." again.  The problem with doing this way is that the numbering starts over with the new page.

I really need to be able to create from fields for document with Nitro PDF Pro for Mac
