"Page View" settings, Thumbnails & Bookmarks

For the default page view settings, I would like to have the Thumbnails window open by default.  It would also be nice if the width of the Thumbnails window would default to either a certain specified width or to the most recent width set by the user.

I agree.  I found under "Document Properties" you can select the "Initial View" tab, then change Page View: to "Pages pane and page" to set each document to show the thumbnail window the next time you open it; a custom thumbnail width isn't an option here, and the last size used doesn't seem to stick.  So, this might partially help if you are constantly referring back to the same document.  I wish this same setting was available under Preferences > Page View so that it could be the default for each document opened, if it is available under preferences I can't seem to find it.
